About The Position

The Registered Nurse (RN) Case Manager provides comprehensive non-clinical case management services in support of the Vermont Army National Guard (VTARNG) medical readiness mission. This role is responsible for coordinating care, managing medical readiness cases, maintaining compliance with Army medical regulations, and ensuring Soldiers receive appropriate medical support to promote operational readiness and deployability. The RN Case Manager serves as a liaison among Soldiers, military providers, civilian healthcare providers, medical readiness personnel, and unit leadership to ensure timely coordination of care, accurate documentation, and continuity of services. The RN Case Manager manages complex medical readiness cases involving Active Duty and National Guard Service Members with acute, chronic, or potentially disqualifying medical conditions. The position focuses on coordinating medical care, maintaining compliance with military health system requirements, supporting Medical Evaluation Board (MEB) and Physical Evaluation Board (PEB) processes, and ensuring accurate documentation within military medical systems such as MEDCHART and eCase. The RN Case Manager is expected to provide proactive case coordination, facilitate treatment planning, track medical readiness compliance, and educate Soldiers regarding healthcare resources, medical documentation requirements, and readiness responsibilities. The role requires strong organizational skills, clinical knowledge, communication abilities, and experience working within regulated healthcare environments.

Requirements

  • Graduate of an accredited nursing program.
  • Current, active, and unrestricted Registered Nurse (RN) license.
  • Multi-state Nurse Licensure Compact (NLC) license with privileges to practice in Vermont accepted.
  • Minimum of two (2) years of clinical nursing or case management experience.
  • Knowledge of military medical readiness programs and healthcare coordination processes.
  • Strong understanding of HIPAA, Privacy Act, and healthcare confidentiality requirements.
  • Excellent organizational and time management skills.
  • Ability to manage multiple complex cases simultaneously.
  • Strong written and verbal communication skills.
  • Ability to work collaboratively with interdisciplinary healthcare teams.
  • Proficiency with electronic medical record systems and Microsoft Office applications.
  • Ability to maintain accurate documentation and meet strict timelines.
  • Ability to work independently in a fast-paced operational environment.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred but not required.
  • Experience coordinating care for patients with complex medical conditions preferred.
  • Prior military healthcare or medical readiness experience highly preferred.
  • Experience using military health systems such as MEDCHART, MEDPROS, MHS GENESIS, or AHLTA preferred.
  • Certified Case Manager (CCM) certification preferred but not required.

Responsibilities

  • Establish and maintain eCase profiles for Soldiers referred by providers within one business day of referral.
  • Coordinate all aspects of non-clinical case management services for assigned Soldiers.
  • Track Soldier medical care from initial identification of a condition through return to medical readiness or administrative separation.
  • Coordinate treatment plans with military and civilian healthcare facilities.
  • Request profile examinations, specialty evaluations, and audiology exams within required timelines.
  • Maintain automated referrals in MEDCHART and monitor case adjudication status.
  • Prepare Soldier records for Medical Evaluation Boards (MEB) and Physical Evaluation Boards (PEB).
  • Review and reconcile hardcopy and electronic case records to ensure completeness and accuracy prior to board review.
  • Support and attend Soldier Readiness Processing (SRP), Periodic Health Assessment (PHA), mobilization, and readiness events as required.
  • Assist with surge and contingency operations during high-volume readiness events.
  • Monitor and validate Medical Readiness Classification (MRC3) and Dental Readiness Classification (DRC3) profiles.
  • Ensure Soldiers with qualifying profiles are accurately entered into eCase systems.
  • Participate in monthly case reviews with VTARNG leadership and health services personnel.
  • Prepare records for mobilization and deployment operations within required timelines.
  • Maintain complete, accurate, and timely medical documentation in eCase, MEDCHART, and associated systems.
  • Document all Soldier encounters, communications, and received medical records within three business days.
  • Upload medical documents into the Health Readiness Record (HRR) and other required systems.
  • Ensure all records are maintained in accordance with AR 40-66 and applicable Army medical regulations.
  • Maintain HIPAA compliance and safeguard all Personally Identifiable Information (PII), Protected Health Information (PHI), and Controlled Unclassified Information (CUI).
  • Ensure compliance with applicable regulations including AR 40-35, AR 40-66, AR 40-501, and AR 40-502.
  • Communicate with Soldiers regarding medical documentation requirements, appointments, treatment plans, and readiness obligations.
  • Educate Soldiers on available medical and dental readiness resources.
  • Coordinate with unit leadership regarding scheduled appointments and follow-up requirements.
  • Obtain releases of information and facilitate coordination with civilian providers and primary care providers (PCPs).
  • Serve as a liaison between military healthcare providers, command teams, and Soldiers to support continuity of care and operational readiness.
