Registered Nurse Case Manager (RN)
About The Position
Bristol Hospice is seeking dedicated RN Case Managers who are passionate about walking beside their patients through every step of the hospice journey. This role involves providing compassionate, whole-person care, acting as an advocate, companion, and comfort to patients and their families. Bristol Hospice is a nationwide industry leader committed to a family-centered approach in hospice services, dedicated to treating all patients and families with the highest level of compassion, respect, and dignity. The schedule is Monday - Friday, 8 AM - 5 PM, with rotating holidays, covering Tremonton to Bountiful.
Requirements
- A valid Registered Nurse license in good standing in the state you will work in.
- A valid driver’s license and reliable transportation.
- CPR certification (current).
- Strong observational and communication skills—both written and verbal.
- A flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities
- Develop and implement individualized care plans based on current and prior medical history.
- Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
- Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
- Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
- Administer physician-ordered medications and treatments with skill and empathy.
- Educate patients and their families on care techniques and symptom management.
- Track and communicate changes in patient condition, ensuring timely and effective responses.
- Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
- Contribute to other supportive duties as part of a compassionate, mission-driven team.
- Perform additional duties as assigned.
