The Hospice Registered Nurse Case Manager role involves conducting comprehensive patient assessments, including vital signs and medical history review. This position requires travel to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, informational visits, and consulting with physicians regarding changes in patient condition. The role also involves educating patients and families on treatment plans, medication management, and health maintenance strategies, as well as submitting clinical notes promptly. Participation in IDG and case conferences, performing on-call duties, and supervising hospice aides are also key aspects of this role. The RNCM will develop and implement the initial plan of care and provide care plans to the Hospice Aide and LPN as part of the IDG team.
