About The Position
National Church Residences Home Health division in Southern Ohio is seeking a Hospice RN Case Manager to join their team of healthcare professionals. This agency is owned and operated by National Church Residences, a leading provider of affordable senior housing and healthcare services. The ideal candidate will share the vision of advancing better living and care for seniors.
Requirements
- Degree in nursing field as a registered nurse from an approved school of nursing.
- Minimum of two years as an RN.

- Home health/hospice experience preferred.

- Provide case management services as a Hospice RN.
