Requirements
- Valid and active Registered Nurse (RN) license.
- Strong clinical knowledge and ability to assess patient needs comprehensively.
- Excellent communication, negotiation, and interpersonal skills.
- Ability to collaborate effectively with multidisciplinary teams.
- Detail-oriented with strong organizational and documentation skills.
- Familiarity with healthcare regulations, insurance processes, and community resources.
- Compassionate, patient-focused approach to care.

Responsibilities
- Assess patients’ healthcare needs, develop individualized care plans, and coordinate services across multiple providers.
- Serve as a primary point of contact for patients and families, providing education, support, and advocacy.
- Collaborate with physicians, nursing staff, social workers, and community resources to ensure comprehensive patient care.
- Monitor patient progress, evaluate outcomes, and revise care plans as necessary.
- Facilitate discharge planning and promote smooth transitions across care settings.
- Maintain accurate and thorough documentation of case management activities and patient interactions.
- Ensure compliance with regulatory requirements and organizational policies.
- Use clinical judgment to identify potential barriers to care and implement strategies to overcome them.
