About The Position

The Registered Nurse - Case Manager assesses and evaluates patients’ care needs and drives discharge planning. The role coordinates services to ensure patients are placed at an appropriate level of care, and ensures treatment plans are initiated, evaluated, and re-evaluated daily. The nurse follows patient progress and lack of progress, monitors length of stay, and identifies barriers to discharge. The role also follows Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.

This is a Monday–Friday assignment with no regularly scheduled weekends or call. Floating may occur within the facility and scope of practice as needed. The position accepts local candidates at the same rate. Full compliance is required to start on time; otherwise, the start may be pushed back by two weeks because of the bi-weekly orientation schedule. Non-billable orientation hours are determined by contract length.

Requirements

• At least 3 years of minimum experience.
• The unit does not accept first-time travelers.
• Full compliance is required to start on time.
