About The Position

The Hospice Registered Nurse Case Manager (RNCM) role involves conducting comprehensive patient assessments, including vital signs and medical history review. The RNCM will travel to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. This role coordinates hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, informational visits, and consulting with physicians regarding changes in patient condition. The RNCM will educate patients and families on treatment plans, medication management, and health maintenance strategies, and submit complete, accurate clinical notes promptly. The role also involves participating in IDG and case conferences, performing on-call duties, developing and implementing the plan of care, supervising and evaluating the care given by the Hospice Aide, and providing the care plan to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned.

Requirements

  • Licensed Registered Nurse
  • BLS Certified
  • Comfortable working in both private homes and facility settings.
  • Valid Driver's License and Auto Insurance Coverage
  • Reliable transportation
  • A minimum of 1 year of Clinical experience

Responsibilities

  • Conduct comprehensive patient assessments, including vital signs and medical history review.
  • Travel to patients' residences to conduct visits.
  • Develop individualized care plans in collaboration with multidisciplinary teams.
  • Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
  • Perform evaluations for hospice care along with informational visits as needed.
  • Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
  • Educate patients and families on treatment plans, medication management, and health maintenance strategies.
  • Submit complete, accurate, and relevant clinical notes promptly.
  • Participate in IDG and case conferences.
  • Performs on-call duties per policy and as needed.
  • Conducts an initial and ongoing comprehensive assessment of the patients.
  • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
  • Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.
  • Provide care plan to Hospice Aide and LPN as part of the IDG team.
  • All other duties as assigned.
