Registered Nurse Case Manager - Hospice

ABODE HOSPICE is seeking a passionate, dedicated Hospice RN to join our team in Colorado Springs, CO. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. As a RN Case Manager, you will assess/ monitor physical, emotional, and psychological needs of patients, create hospice care plans that align with the patient's wishes and goals, direct nursing care by administering medications, treatments, and interventions, provide pain and symptom management, educate and support the patient’s family and caregivers, collaborate with the interdisciplinary team, and maintain accurate and timely documentation. Participation in on-call rotation is required as per local branch needs. Requirements
- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year nursing experience
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
Nice To Haves
- 3+ years preferred nursing experience
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
Responsibilities
