About The Position

Haven Hospice is seeking a passionate, dedicated Hospice RN to join their team in Lake City, FL. This role provides expert, patient-centered care at a critical time in people’s lives. The position offers a supportive, fulfilling environment where skills and empathy shine. The Registered Nurse Case Manager will assess and monitor patients' physical, emotional, and psychological needs, create hospice care plans aligned with patient wishes, and direct nursing care including administering medications, treatments, and interventions. Responsibilities also include providing pain and symptom management, educating and supporting the patient's family and caregivers, collaborating with an interdisciplinary team, and maintaining accurate documentation. Participation in on-call rotation may be required.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Nice To Haves

• 3+ years preferred nursing experience
• Experience in hospice or a similar setting preferred

Responsibilities
