About The Position

JourneyCare Hospice is seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Evanston area. They offer manageable caseloads, strong interdisciplinary support, and prioritize quality of care over visit volume. Nurses can expect a consistent Monday-Friday schedule with only one weekend rotation every 12 weeks, and a supportive leadership team. The role involves providing skilled nursing care, coordinating with physicians and the interdisciplinary team, and offering end-of-life support to patients and families.

Requirements

• Graduate of an accredited RN program
• Active Illinois RN license
• Current CPR and First Aid certification
• Hospice or home health experience preferred
• Strong clinical judgment and communication skills
• Valid driver’s license and proof of insurance

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Maintain timely, accurate documentation
• Supervise Home Health Aides in compliance with regulations
