The Hospice Registered Nurse Case Manager role involves conducting comprehensive patient assessments, including vital signs and medical history review. This position requires travel to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, informational visits, and consulting with physicians regarding patient condition changes. The role also involves educating patients and families on treatment plans and medication management, submitting clinical notes promptly, and participating in IDG and case conferences. On-call duties are also part of the role, along with supervising and evaluating the care given by the Hospice Aide and providing care plans to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned will also be part of the role.
