RN (Registered Nurse) Case Manager, Hospice
About The Position
Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. They will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life. Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
Requirements
- Active Kansas or Compact RN License
- Valid driver’s license and auto liability insurance
- Hospice experience preferred
Responsibilities
- Provide nursing care, education, and support to patients and families
- Complete initial and ongoing assessments
- Coordinate care with physicians and the hospice interdisciplinary team
- Develop and update patient-centered plans of care
- Provide guidance with healthcare decisions
