About The Position

The Registered Nurse (RN) in Case Management is responsible for coordinating the overall interdisciplinary plan of care for patients, from admission to discharge. This role involves creating a plan for care across the continuum, integrating patient and family preferences and values, and monitoring patient medical necessity and level of care through assessments and ongoing evaluations. The RN advocates for resources and removal of barriers, maintains communication with supervisors and care transition team members, and serves as a resource for medical record documentation and level of care recommendations for post-discharge care.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.
  • Diploma from an accredited school/college of nursing and professional licensure at time of hire required.

Nice To Haves

  • 1 year Acute Care bedside experience
  • 1 year Managed Care Prior Authorization experience
  • 1 year Utilization Management/Review experience
  • CCM, ACM, or ANCC certification

Responsibilities

  • Creates plan for care across the continuum, integrating patient/family preferences and values.
  • Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
  • Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
