Requirements

  • Reliable transportation with current insurance and a valid driver’s license.
  • Must possess excellent written and verbal communication skills and be able to interact effectively with a diverse audience including clients, families, care providers and other professionals.
  • Must possess impeccable follow up and attention to detail and be able to exercise initiative, problem solve, apply sound judgment.
  • Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families.
  • Pass a criminal background check.

Nice To Haves

  • Two years of Registered Nurse experience preferred but not required in home care, home health or related field
  • Passionate about helping others live with dignity.
  • Must present a positive and professional business image.

Responsibilities

  • Meeting potential and current clients in their homes to listen to their stories, determine their needs, and establish a plan of care during an initial assessment.
  • Managing the coordination of care and maintaining compliance with policies and procedures and all state home care rules and regulations.
  • Provides home visits on an as needed basis or every 90 days to monitor any changes in client condition.
  • Oversee the Caregivers providing care to our wonderful clients.
  • Assist in New Hire Orientation and Training of new Caregivers, as well as perform skills assessment on New Hires.
  • Observe Caregivers in the home and assist in employee reviews.
