About The Position

The Nurse Care Manager utilizes expert clinical skills and a disease specific knowledge base to assist identified patients through the continuum of care in an effort to provide appropriate, quality care in a cost-effective manner. The Nurse Care Manager conducts daily assessments to identify high risk patients and manages their care during the acute care episode. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Requirements

• Graduate of an accredited school of nursing required.
• Current licensure as a Registered Nurse in the State of New Jersey required.
• BLSHCP Certification required 6 months from date of hire or transfer.
• 5-7 years clinical experience required.
• Excellent interpersonal, written communication skills and computer literacy required.
• Analytical and delegation skills required.

Nice To Haves

• Bachelor's in Nursing preferred.
• Prior case management/insurance management experience preferred.
• Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
• For some specialties, a Master's degree in Nursing may be required, with an advanced practice nurse certification.
