Requirements

Active Registered Nurse (RN) license in NJ
Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
Solid clinical assessment skills and a passion for problem-solving
CPR/First Aid certification
Valid driver’s license
Warm, professional communication skills to build trust with families and field staff

Responsibilities

Complete in-home RN assessments and craft individualized Care Plans with client/family approval.
Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
Track vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
Coordinate with physicians, therapists, and schedulers to ensure seamless and client-centered care.
Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
