About The Position

As a Nurse Care Manager (NCM), you’ll be at the heart of providing personalized care to your patients, helping them navigate their health journeys and achieve their best possible outcomes. You’ll work across various healthcare settings to integrate care, coordinate treatment plans, and empower patients to manage their illnesses effectively. In this role, you’ll collaborate with a dedicated healthcare team, applying evidence-based practices to ensure every patient gets the care they deserve. Requirements

• BSN (or equivalent combination of ADN and relevant experience).
• Current Missouri RN license.
• Current BCLS (Basic Life Support) certification.
• A minimum of three years working in a healthcare setting.
• Strong written and verbal communication skills—you’ll need to effectively coordinate with patients and the healthcare team.
• Proficient in Microsoft Word, Excel, and PowerPoint.
• Must be able to read and speak English fluently.

Nice To Haves

• Any experience working in community health is a plus.
• Ability to speak multiple languages is a bonus!

Responsibilities

• Design, implement, and evaluate treatment plans by collaborating with your healthcare team, ensuring each patient receives the best care at every stage of their journey.
• Work with your patients to help them manage their illnesses and navigate the healthcare system, supporting them to achieve optimal health outcomes.
• Provide and manage care for Health Home enrollees, tracking their healthcare outcomes and making adjustments to treatment plans as needed.
• Actively participate in and coordinate patient care with other team members, fostering a holistic approach to treatment and recovery.
• Use evidence-based information to drive care decisions, ensuring that every patient receives high-quality, up-to-date treatment.
