Requirements

Bachelors of Science in Nursing (BSN) from an accredited college of nursing required, or obtained within 5 years from date of hire for nurses with an Associate Degree in Nursing (ADN).
Successful completion of annual mandatory education requirements.
Minimum of two years of relevant clinical experience.
Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
Ability to multi-task and prioritize work assignments.
Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
Ability to think critically and problem solve.
Excellent negotiation skills, with the ability to address complex issues with physicians and staff across multiple departments.

Nice To Haves

Certification(s) in area of specialty preferred.
From National Certification Corporation (NCC) within 1 year of eligibility.
Case Management certification preferred.

Responsibilities

Provides appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria.
Performs an initial and ongoing assessment to determine the patient's needs, including data from the patient/significant other(s). Prioritizes data collection and initiates discharge planning upon admission.
Develops/reviews/revises and individualizes the plan of care to address the patient's nursing care needs for optimum patient care outcomes, collaborating with providers and other clinical disciplines.
Implements interventions that are consistent with the established plan of care in a safe and appropriate manner, following nursing protocols and standards of care.
Performs technical skills competently and independently to assure patient safety, and is accountable to learn new skills and equipment.
Maintains professional behavior in attitude, demeanor, and interactions to achieve the organization's mission, communicating effectively and maintaining a professional appearance and work environment.
Initiates action to correct, prevent, and/or minimize risks to the patient, family, or staff/employees, completing appropriate reports for risks and communicating unresolved issues.
Acts professionally responsible for human and other resources, charging accurately for supplies and equipment, and understanding length of stay goals.
Coordinates care by collaborating with physician, interdisciplinary team, and patient/family to develop comprehensive discharge plans, establishing effective relationships, and documenting accurately.
Participates in Performance Improvement activities, assists with update/development of critical path and standing order sets, and participates in hospital-wide committees and initiatives as assigned.
