The Community Health Centers of Benton and Linn Counties are seeking a Registered Nurse Care Coordinator to join their team. Nurses in this role work in a patient-centered environment with professionals dedicated to preventive, team-based care. Registered Nurses provide a broad range of nursing services in clinical, community, and home settings, focusing on disease prevention, treatment, and health policy implementation. They utilize critical thinking and nursing process skills to screen, assess, plan, deliver, and evaluate health interventions for individuals and populations. This position involves triaging clients to assess their health status for immediate care, clinical appointments, education for home care, or referrals to resources. The role requires demonstrating knowledge-based care to assigned clients and programs in accordance with standards of practice, the nursing practice act, Oregon Administrative Rules, and County and department policies and procedures. Specialized services may be provided within specific programs and at multiple sites according to scope, license, and program standards.
