The Registered Nurse Care Coordinator (RN) for the Cardiology Clinic plays a crucial role in ensuring continuity of care for patients. This position involves developing strong relationships with patients and their multidisciplinary teams to facilitate and navigate subsequent treatment and follow-up. The RN will conduct nursing assessments to identify individual patient needs, collaborate with the multidisciplinary team to develop interdisciplinary care plans, and provide education, address psychosocial needs, advocate for patients, and identify barriers to care. The role also involves navigating patients through diagnosis, treatment, and follow-up across the care continuum, providing expert clinical advice, and coordinating timely, seamless, evidence-based care to achieve optimal patient outcomes. Additionally, the RN will evaluate and design patient education materials, provide education and outreach programs, and assist leadership in developing program vision, goals, and objectives. This role also contributes to the development and maintenance of quality programs focused on best practice patient care outcomes, quality of life, and enhanced patient loyalty. The incumbent must demonstrate knowledge and skills appropriate for the age of the patients served, including understanding growth and development principles and assessing data reflective of patient status.
