About The Position

MercyOne Genesis is seeking a Registered Nurse to join their Care Coordination Team. This role focuses on improving community health by ensuring patients' progress through acute care episodes, post-discharge, and safe, efficient, and cost-effective discharge planning. The RN will collaborate with physicians and the multidisciplinary team to meet patient needs, coordinate care across the continuum, ensure appropriate reimbursement, and optimize resource utilization. They will also serve as a liaison between the hospital and community agencies for clinical and referral information.

Requirements

• Registered Nurse in Iowa or Illinois at time of hire, second state required within 1 year.
• More than 3 years experience required
• Basic Life Support Training

Nice To Haves

• Bachelor's Degree in Nursing, or related field
• Certification in Case Management, Professional Utilization Review or Managed Care
• DRG/CPT knowledge
• Interqual Admission/Continued Stay criteria knowledge

Responsibilities

• Concurrently and retrospectively ensures the patient's progress in the acute episode of care through post discharge and is quality driven while being efficient, cost effective, and safe discharge plan
• Works with physicians to facilitate effective and efficient transitions through the process of hospitalization.
• Works collaboratively with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by payers, and that resources are efficiently and effectively utilized
• Serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information
