About The Position

This role involves championing patient success by proactively implementing, evaluating, and modifying individualized patient care plans. The nurse will act as a patient advocate, continuously monitoring, documenting, and communicating changes in patient status. They will serve as the central coordinator for care across medical disciplines and meticulously carry out physician and advanced practice provider orders. Additionally, the nurse will assess patient discharge needs and collaborate with the multidisciplinary team for a safe transition.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Oklahoma Board of Nursing required.
Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.

Responsibilities

Proactively implement, evaluate, and modify individualized patient care plans to achieve the best possible outcomes.
Continuously monitor, accurately document, and promptly communicate changes in patient status to the care team.
Serve as the central coordinator, connecting and facilitating seamless, well-orchestrated care across all medical disciplines.
Meticulously carry out physician and advanced practice provider orders, along with specialized nursing protocols.
Holistically assess patient discharge needs and proactively collaborate with the multidisciplinary team to plan for a safe, successful transition home or to the next level of care.
