About The Position

This position functions on the multidisciplinary team as an advocate, educator and counselor for breast disease oncology patients. Responsible for ensuring all patients with a breast disease diagnosis receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team. Serves as a clinical resource with expertise in breast disease oncology care management. Serves as a liaison throughout the facility and in the community regarding services provided for this unique patient population. Provides expert nursing care, which includes consultation, direct clinical practice, education, and research. Through the application of evidenced-based decision making, the RN Breast Navigator serves as an educator, researcher and clinical mentor to all members of the health care team.

Requirements

  • Bachelors Degree Nursing Required
  • 1 Year of previous clinical oncology experience.
  • Strong problem-solving skills
  • Ability to function in an organized and time conscious manner
  • Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Required
  • Certified Breast Care Nurse (CBCN®) - Oncology Nursing Certification Corporation (ONCC) Required
  • AHA Heartsaver CPR - American Heart Association (AHA) Required

Responsibilities

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
  • Implements a nurse navigator model that provides education and information to the patient and family, helping to make the care seamless, continuous and comprehensive.
  • Initiates and documents patient teaching including family and significant others based on assessment of needs.
  • Reinforces education on disease, prevention, detection, diagnosis, multidisciplinary treatments, side effects, adverse reactions, and reportable sign signs/symptoms, survivorship based on multidisciplinary plan.
  • Utilizes appropriate patient education documentation modality.
  • Provides ongoing education to patients and family regarding breast health and disease.
  • Acts as the single point of contact for breast cancer patients and families.
  • Identifies patient and family needs as it relates to continuity and efficiency of breast care.
  • Coordinates and schedules tests, procedures, appointments, and treatments as needed.
  • May accompany patient to initial appointments diagnosis and treatment planning period.
  • Assists in the development of a treatment plan that is understandable and feasible to the patient.
  • Acts as the point person for patients and all other care providers.
  • Enable communication by acting as a liaison between the patient and family and the physicians, other healthcare team members to coordinate integrated care across the continuum.
  • Collaborates with physicians to expedite multi-specialty discussions regarding the treatment plan.
  • Supports the patient during difficult decision-making periods.
  • Directs/refers patients and families to available supportive care services, including dietitians, social workers, financial counselors, and community resources.
  • Assists eligible patients access appropriate clinical trials
  • Partners with patients, families, the interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate and exemplary, interdisciplinary care enhancing patient experience.
  • Communicates with all members of the healthcare team about patient/family needs and concerns, as appropriate.
  • Partners with other healthcare disciplines to ensure timely appointments, results reporting, and communication on patient care and follow up.
  • Mentors staff and provides informal staff education and role modeling of patient management activities related to breast health.
  • Initiates and performs ongoing review of policies related to service provided.
  • Updates or writes new policies to enhance professional practice where appropriate.
  • Develops and/or coordinate development of patient education materials, clinical pathways, and quality studies.
  • Promotes process improvement by identifying improvement opportunities, leading quality initiatives and implementing change.
  • Participates in research and quality improvement projects related to ensuring patient access to care.
  • Promotes evidence-based practice in nursing by integrating nursing research results into practice and adopting the use of clinical, educational and managerial research findings.
  • Fosters process improvement by identifying improvement opportunities, leading quality initiatives and implementing change.
  • Identifies important data elements for tracking organizational performance relative to caring for patients with breast disease and responsible for periodic auditing and report creation and presentation.
  • Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as staff education with the Clinical Educator.
  • Promotes community education and increases prevention awareness and early-detection screening programs.
  • Maintains clinical expertise through participation in their professional organization, attending related seminars and conferences and maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature.
