About The Position

Plans, provides, and/or supervises the nursing care of patients in an efficient and caring manner. Coordinates care planning with other caregivers and with other areas, adheres to universal precautions, and is compliant with infection control policies. Works independently with minimal supervision. This role may also include duties of a Chemical Dependency Counselor (CDC), assisting the medical director with patient rounds, providing assessment and therapeutic counseling to chemically dependent clients using a bio-psychosocial approach in an outpatient setting, educating clients and families regarding the disease process for chemical dependency, conducting individual and group therapy, and leading group discussions. The CDC addendum also involves responsibility for the care planning of a caseload of clients with chemical dependency issues, assessing patient needs, and updating the medical director.

Requirements

  • Current Ohio RN License
  • ADN or BSN
  • CPR certification or obtain within 30 days of employment.
  • Phlebotomy classes within 90 days of employment departmental provided training.
  • Ability to read, write and explain or describe moderately complex information to other hospital staff.
  • Attention to detail.
  • Ability to accurately take and record measurements or readings.
  • Ability to provide patient care in a warm and friendly manner.
  • Demonstrates competence in age-specific patient care as documented by the attached checklist.
  • Aggression Management System within six months of employment.
  • Must be licensed by the Ohio Chemical Dependency Professionals Board with a minimum of a Chemical Dependency Counselor Assistant certification within 6 months of employment (CDC addendum).
  • Skills to provide care based on physical, psychosocial, educational, safety, and related criteria appropriate for the patient age (CDC addendum).
  • Knowledge of chemical dependency processes and treatment modalities (CDC addendum).
  • Demonstrated skills appropriate for ASAM levels of care, SASSI, CIWA, COWS and DSM V usage (CDC addendum).
  • Demonstrated knowledge base of chemical dependency/addiction to educate clients, families and community (CDC addendum).
  • Must have excellent written and verbal skills to facilitate working with various multidisciplinary team members and community agencies (CDC addendum).
  • AMS certification within 30 days of employment (CDC addendum).

Responsibilities

  • Appropriately assumes responsibility for group of patients assigned.
  • Demonstrates appropriate administration of medication.
  • Provides/facilitates patient/family education.
  • Involves the patient and family/significant other in any decision making related to the patient’s plan of care.
  • Recognizes and reports signs of abuse or domestic violence.
  • Displays ability and uses critical thinking skills to set priorities for patient care, self, and others.
  • Proficient in vital sign assessment, including pain.
  • Demonstrates competence in age specific patient care as documented on the attached checklist.
  • Delegates appropriate tasks based on acuity of patient and skill level of resource staff.
  • Completes billing charges/sheets daily (CDC addendum).
  • Utilizes historic and current data in assessing patient needs (CDC addendum).
  • Utilizes effective interviewing skills and supportive counseling skills (CDC addendum).
  • Provides individual and group, crisis intervention, and family clinical interventions with chemically dependent clients (CDC addendum).
  • Acts as client advocate, mediator, and facilitator (CDC addendum).
  • Assists with orientation of new staff and/or students (CDC addendum).
  • Provides community services including development of programs (CDC addendum).
  • Cross-trains as appropriate to credentials, licensure and scope of practice and department need (CDC addendum).
  • Develops care plans based on assessment (CDC addendum).
  • Conducts bio-psych social assessments, including nursing assessments (CDC addendum).
  • Documents patient’s progress, care plans, assessments, etc. in the Electronic Medical Record (CDC addendum).
  • Proficient in vital sign assessment and phlebotomy (CDC addendum).
  • Carries out physician orders (CDC addendum).
  • Calls in medication orders (CDC addendum).
  • Performs other functions within RN scope of practice, as needed (CDC addendum).
