Provides care for a specific patient population with various types of chronic wounds and their advanced wound care treatments. Serves as a resource for other health care professionals, patients, families and the community. Requirements

Associate degree in Nursing.
Two (2) years nursing experience.
Experience in skin and wound care.
Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

Nice To Haves

Bachelor's degree in Nursing.
Wound Vac experience.
Certification in Wound and Ostomy.

Responsibilities

Under the guidance of a physician and/or a Certified Wound Care Clinician, perform assessment/reassessment of the physical and educational needs of patients with any chronic wound needing advanced wound care techniques.
Communicate patient needs to appropriate members of the wound care team.
Provide discharge teaching as required.
Assist with referrals for studies, labs, procedures, and other physicians.
Under the guidance of a physician and/or Certified Wound Care Clinician, formulate, implement and evaluate an individualized plan of care based on patient needs.
Through consultation and collaboration with the members of the wound care team, coordinate care in an efficient, cost-effective manner using knowledge and understanding of current advanced wound care modalities and practices.
Assist with education of patients and their caregivers on current evidence-based advanced wound care modalities and practices.
Assist with promoting knowledge of healthy practices in area of expertise throughout the community through education.
Identify actual/potential issues of resource management/patient care; work with wound care team to identify cost-effective alternatives; facilitate discharge.
Keep documentation via EMR current including accurate documentation of products and facility charges.
Maintain a safe, comfortable, clean and therapeutic environment for patients and families per established departmental policies.
May perform hyperbaric oxygen therapy related duties, as needed, after completing the required hyperbaric training.
Perform other duties as assigned.
