About The Position

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is the second largest multi-specialty group in New Hampshire, serving the greater Nashua community. The clinical float pool comprises Registered Nurses (RN), Licensed Practical Nurses (LPN), and Medical Assistants (MA) who provide temporary coverage for staffing needs across multiple sites. Float pool staff undertake various clinical functions in our family practices located throughout Southern New Hampshire.

Requirements

• Current NH Registered Nursing (RN) licensure is required.
• Current BLS/CPR training.
• Prior work experience in a healthcare setting is required.

Responsibilities

• Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.
• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities.
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
