About The Position
Grace Community Care & Homes is developing its Adult Day Programs department and seeks a Registered Nurse Adult Day Health. The role will contribute to person-centered services, reliable operations, and accurate documentation. This is a planned role. The start date and duties are contingent on applicable program approvals, funding, credentialing, and staffing readiness. Services may begin only within the approved scope. The position is based in New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. Weekly hours and the start date will be discussed during hiring; clinical services begin only at approved locations and within authorized scope.
Requirements
- Active New Jersey RN license.
- Relevant adult-care experience.
- Role-appropriate clinical skills and training for the licensed adult day health model.
- Provide relevant credentials, experience, availability, and a resume.
- Final regulatory eligibility will be verified before appointment or service delivery.
Responsibilities
- Provide nursing assessment, monitoring, care planning, medication services within scope, and patient education.
- Coordinate with the interdisciplinary day team.
- Work collaboratively with department leadership, intake, credentialing, billing, and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review, and quality improvement.
