About The Position

This full-time Registered Nurse position focuses on Admissions, Discharge, and Transitions of Care. The role involves utilizing the nursing process to manage patient needs during admission, transfer, and discharge. Key responsibilities include reviewing orders, communicating with patient and family, collaborating with multidisciplinary teams for discharge planning, ensuring patients are ready for discharge, providing patient and family education, and facilitating follow-up appointments. The nurse will also conduct post-discharge follow-up calls and serve as a liaison between medical teams and patients/families. The position aims to improve quality metrics such as reducing emergency department overuse, increasing Press Ganey scores, decreasing readmissions, and optimizing length of stay. Professional accountability includes maintaining confidentiality, adhering to dress code, attending meetings, maintaining clinical competency, and complying with policies and procedures. The role may require floating to other units as needed and serving as a mentor.

Requirements

  • Bachelor’s degree required.
  • Current/valid Florida nursing license.
  • CPR/BLS required.
  • ACLS required.
  • 3 years of recent nursing experience required.
  • Strong interpersonal communication skills.
  • Proven track record of building relationships with physicians, direct reports, and key staff.
  • Moderate to advanced computer literacy.
  • Maintains clinical competency.
  • Maintains professional certifications/licensure in active status and good standing.
  • Practices within the scope and guidelines set forth by the Florida Board of Nursing.

Responsibilities

  • Utilize the nursing process to anticipate and plan for patient and family needs at admission, transfer, and discharge.
  • Review all orders on new admissions, transfers, and discharges.
  • Communicate patient changes and condition with assigned team members.
  • Communicate with patient/family within 24 to 72 hours of admission.
  • Attend daily interdisciplinary team rounds.
  • Collaborate with Primary RN, case managers/social services during the patient's stay.
  • Coordinate interdisciplinary discharge planning for a specific patient population.
  • Facilitate access to outpatient resources for patients.
  • Work with APRNs and other Providers to ensure patients are ready for discharge by noon.
  • Demonstrate commitment to meeting the learning needs of patients and families.
  • Utilize appropriate resources to assist in meeting patient learning needs and achieve positive patient outcomes.
  • Assist the patient/family in maximizing their sense of control and actively participating in their recovery.
  • Support the patient discharge process, including medication reconciliation, finalizing discharge orders, completing the discharge summary, and providing patient/family education.
  • Validate patient/family education and comprehension before discharge.
  • Facilitate all follow-up appointments and ensure ordered tests and procedures are scheduled before discharge.
  • Communicate with patient/family 24 hours post-discharge.
  • Conduct patient follow-up at 3, 7, 14, and up to 30 days post-discharge as needed.
  • Serve as a liaison between medical and surgical teams and patients and their families post-discharge.
  • Provide care appropriate to the age and needs of the patients served on the assigned unit.
  • Serve as a direct care provider, as appropriate, to ensure positive outcomes.
  • Maintain a safe and clean environment that promotes the safety of patients and their families.
  • Comply with regulatory standards, including National Patient Safety Goals.
  • Demonstrate creativity and innovation in approach to complex problem solving.
  • Consistently maintain complete confidentiality of all medical, financial, employee, computer, and other sensitive materials.
  • Maintain a professional appearance and adhere to the proper dress code.
  • Always wear the hospital ID badge and locator, and promote the standard of appropriate identification for peers and staff.
  • Attend meetings as required.
  • Serve as a role model and consistently display a professional demeanor.
  • Maintain clinical competency.
  • Demonstrate professional independence in the practice of nursing.
  • Consult internal/external experts when appropriate.
  • Maintain documentation of continuing education requirements.
  • Maintain certification (CPR/BLS and ACLS).
  • Attend all mandatory in-services and complete all computer-based modules as assigned.
  • Actively participate in shared governance councils and professional organizations.
  • Actively support and participate in performance improvement activities.
  • Seek/accept training to learn new skills and maintain current competencies.
  • Maintain professional certifications/licensure in active status and good standing.
  • Practice within the scope and guidelines set forth by the Florida Board of Nursing.
  • Attempt to resolve patient, physician, and visitor complaints and refer unresolved issues to managers.
  • Demonstrate dependability/reliability by promptly reporting to work when scheduled.
  • Float to other units in the role of ADT, as necessary.
  • Serve as a mentor and role model.
  • Comply with UM, UHealth/UMHC, and department policies and procedures.
  • Other duties as assigned.
