This full-time Registered Nurse position focuses on Admissions, Discharge, and Transitions of Care. The role involves utilizing the nursing process to manage patient needs during admission, transfer, and discharge. Key responsibilities include reviewing orders, communicating patient status with the healthcare team, collaborating with interdisciplinary teams for discharge planning, and ensuring patients are prepared for discharge by 12 noon. The nurse will also be responsible for medication reconciliation, finalizing discharge orders, providing patient and family education, facilitating follow-up appointments, and conducting post-discharge follow-up calls. The position requires demonstrating age-appropriate care, maintaining a safe environment, and complying with regulatory standards. The role also emphasizes professional accountability, including maintaining confidentiality, professional appearance, and continuing education. The Registered Nurse will act as a liaison for patients and families post-discharge and may be required to float to other units as needed. This position reports to the ADT Supervisor and has no supervisory duties.
