About The Position

This full-time Registered Nurse position focuses on Admissions, Discharge, and Transitions of Care. The role involves utilizing the nursing process to manage patient needs during admission, transfer, and discharge. Key responsibilities include reviewing orders, communicating patient status with the healthcare team, collaborating with interdisciplinary teams for discharge planning, and ensuring patients are prepared for discharge by 12 noon. The nurse will also be responsible for medication reconciliation, finalizing discharge orders, providing patient and family education, facilitating follow-up appointments, and conducting post-discharge follow-up calls. The position requires demonstrating age-appropriate care, maintaining a safe environment, and complying with regulatory standards. The role also emphasizes professional accountability, including maintaining confidentiality, professional appearance, and continuing education. The Registered Nurse will act as a liaison for patients and families post-discharge and may be required to float to other units as needed. This position reports to the ADT Supervisor and has no supervisory duties.

Requirements

Graduate from a nationally accredited school of nursing
Bachelor’s degree required
Current/valid Florida nursing license
BLS/ACLS Provider card required
Three years recent nursing experience required
Strong interpersonal communication skills
Proven track record of building relationships with physicians, direct reports and key staff
Moderate to advanced computer literacy
CPR/BLS and ACLS are required.

Responsibilities

Demonstrates the use of nursing process as a problem-solving model; utilizing their knowledge and experience to anticipate and plan to meet patient and family needs at time of admission, transfer, and discharge.
Reviews all orders on new admission, transfers, and discharges.
Communicates patients changes and condition with the assigned team members as indicated.
Communicates with patient/family within 24 to 72 hours of admission.
Communicates daily with the interdisciplinary team after rounds.
Collaborates with Primary RN, case managers/social services during the patient's stay.
Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population, and facilitates outpatient resources that the patient may need in collaboration with the interdisciplinary team.
Works with NP and other Providers throughout the stay to ensure patient will be ready for discharge by 12 noon.
Demonstrates commitment to meeting the learning needs of patients and families.
Utilizes appropriate resources to assist in meeting those needs and achieve positive patient outcomes.
Assists patient/family to maximize sense of control and actively participate in his/her recovery.
Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders: completes the discharge summary, provides patient/family education, and documents discharge teaching.
Validates patient/family education and comprehension prior to discharge.
Facilitates all follow-up appointments, and ensure ordered tests/procedures are scheduled prior to patients discharge.
Communicates with patient/family 24 hours post discharge and documents encounter in UChart (telephone encounter).
Conducts patient follow up at the following intervals (7, 14 and 30 days) post discharge and as needed.
Documents encounter in UChart (telephone encounter).
Is a liaison between medical/surgical team for patients/family post discharge.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age, and needs of the patients served on the assigned unit.
Serves as a direct care provider, as appropriate, to ensure positive outcomes.
Maintains a safe and clean environment that promotes safety of patients/family throughout all environments of care.
Complies with regulatory standards including National Patient Safety Goals.
Demonstrates creativity and innovation in approach to complex problem solving.
Consistently maintains complete confidentiality of all medical, financial, employee, computer and other sensitive materials, which may violate HIPAA regulations and/or jeopardize the privacy of others.
Maintains professional appearance and adheres to proper dress code.
Wears hospital I.D. badge and locator at all times, and promotes the standard of proper identification for peers and staff.
Attends meetings as required.
