Seneca Health Services is a Certified Community Behavioral Health Center in wild, wonderful West Virginia. We provide responsible, accessible, and progressive behavioral health services. ACT Registered Nurses provide psychiatric nursing services for ACT clients. Conduct activities such as psychiatric assessments, linkage, and advocacy to maintain and support ACT clients in their community placements. Coordinate, train and supervise AMAP certified employees. The overall goal of the ACT team is to reduce the number of inpatient hospitalizations and assist the consumer in developing a comprehensive and successful community placement. Provide high-quality nursing assistance to physicians and other medical professionals while ensuring comprehensive client care and maximizing efficiency. Ensure the delivery of high-quality, compliant, and effective clinical services by reviewing and auditing documentation for accuracy, timeliness, and adherence to regulatory standards.

Requirements

  • Current West Virginia Registered Nurse licensure required.
  • One year experience working with individuals diagnosed with mental illness, intellectual developmental disability and/or substance use disorder preferred.
  • Must be certified or eligible to be certified in WV to provide instruction and supervision of the WV AMAP program.
  • Valid driver’s license and vehicle required.
  • Must pass a Criminal Background Check and Employment Fitness Determination through WVCARES and a drug screen.
  • Demonstrated knowledge of basic computer skills required.
  • Communication and interpersonal skills to effectively interact with employees and others.
  • Organizational skills to manage multiple tasks, prioritize workload, and meet deadlines.
  • Proficient with Microsoft Office (Outlook, Word, Excel).
  • Proficiency in Microsoft Word and Excel.
  • Knowledge of behavioral health and health care regulations and compliance.

Responsibilities

  • Provide staff and client training on medications and other health related issues; coordinate, train and supervise AMAP certified employees.
  • Perform basic health and medical assessments.
  • Coordinate health care provided to ACT clients in the community; provide advocacy for clients at medical appointments; link clients with needed support in the community.
  • Provide psychotropic medication monitoring, tracking, and administration.
  • Provide assessment, treatment planning, and service delivery to ACT clients.
  • Make home visits, administer injections, assess symptoms and response to medication, and oversee prescriptions and medication maintenance.
  • Participate in daily meetings and treatment plan meetings, as required.
  • Provide general support to ensure ADL functions are complete.
  • Works closely with ACT staff to encourage client medication compliance.
  • Assess symptom acuity and coordinates appropriate interventions/supports to prevent hospitalizations/commitments.
  • Coordinate and supervise administration of medication, ensure ongoing inventory of medications, and current physician orders.
  • Monitor and review of MARS, medication storage, and documentation.
  • Provide nursing assistance to physicians and other medical professionals.
  • Provide nursing assessment, as required by medical provider.
  • Review and audit clinical documentation for accuracy and timeliness.
  • Maintain knowledge of Acentra, CCBHC, OHFLAC, BMS and CARF standards, and other regulatory requirements.
  • Dispense sample medications according to medical provider’s orders and documents accordingly.
  • Review patient medication lists, update client history, follow up on referrals to in-house and outside facilities for results and testing for clients.
  • Complete screening for preventative care conditions such as Body Mass Index (BMI), nutrition, physical activity, tobacco and alcohol use, depression, and suicide risk.
  • Provide education on health issues, such as tobacco cessation, alcohol and drug use, diabetes, heart disease, hypertension, medication compliance, etc.
  • Ensure nursing services provided to individuals served are documented in compliance with regulations and standards.
  • Assist with 24/7 on-call nursing coverage.
  • Assure the provision of high-quality services for clients by becoming knowledgeable of and adhering to rules, regulations, and legal requirements and by documenting services provided in accordance with established standards; secure information by adhering to HIPAA and 42 CFR Part 2 regulations and Seneca privacy policies.
  • Utilize Personal Protective Equipment and follow guidelines outlined in Seneca’s Exposure Control Plan.
  • Represent Seneca in a positive manner, including maintaining a positive attitude and performing duties in accordance with Seneca’s Mission and Code of Conduct and Ethical Practice.
