About The Position

Handles sensitive information with the highest degree of integrity and confidentiality. Responsible for planning, implementing, accurately passing medications and evaluating the resident's daily and ongoing needs under the direction and supervision of the Lead RMA, Nursing Supervisor, DON, Unit Coordinator or Designee. Follow the Five Rights of Medication Administration (right patient, medication, dose, route, and time). Verify medication orders and identify discrepancies prior to administration. Observe residents for medication effectiveness and adverse reactions and report findings to the licensed nurse. Completes accurate documentation of the medication administration record (MAR). Maintain proper medication storage, labeling, and security. To monitor individual medications of the residents and ensure adequate supply of medications for residents. Maintains a stocked, clean med cart. Follows proper disposal of medications and narcotics/scheduled drugs. Performs narcotic count at the beginning & end of each shift as outlined in policies & procedures. Performs daily tasks as set forth by the Lead RMA, Nursing Supervisor, DON, Unit Coordinator, Administrator, or Designee. Cooperates fully with all staff in the best interest of the residents and to act as a team member in all areas. Is knowledgeable of the individualized care plan for residents and provides support to the residents according to their care plan. Contributes to the care planning process by providing the care planning staff with specific information and observations of the residents’ needs and preferences. Possesses the ability to follow complex written and oral directions. Communicates and interacts effectively and tactfully with residents, visitors, families, peers and supervisors. Provides resident supervision and/or assistance with activities of daily living, (ADLs): bathing, dressing, grooming, toileting, incontinent care, transferring, feeding, communication, ambulation, and documents ADLs provided by employee. Assists with meal service and clean-up process. Encourages and documents daily nutritional intake. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to Lead RMA, Nursing Supervisor, DON, UC, Administrator or Designee incidents or evidence of resident abuse or violation of residents’ rights. Maintains the comfort, privacy and dignity of residents in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring, homelike environment. Answers residents’ call bells promptly and courteously. Documents on 24 hour report/progress notes/nurses notes. Provides support in any area that may arise that is above and beyond the normal expectations for this position. Assists in maintaining a safe, neat and clean environment; reports environmental deficiencies to appropriate staff person, such as lighting or equipment problems. Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items. Observes residents for changes in condition or behavior and promptly reports these changes to Lead RMA, Nursing Supervisor, DON, UC, Administrator or Designee. Provides care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas. Participates in mandatory periodic training to enhance their skills and maintain the current trends in geriatric care. All staff are required to attend 12 hours of resident centered training and 4 hours of cognitive impairment training annually in accordance with standards for licensed Assisted Living Facilities and infection control training. Provides basic First Aid and CPR when necessary and maintains current certification.

Requirements

  • Graduation from high school or GED
  • Hold a current and valid license as a Registered Nurse or Licensed Practical Nurse or Registered Medication Aide from the Virginia Board of Nursing/Department of Health Professions & maintains without violations/restrictions
  • State Police background check that is free from barrier offenses
  • A current Tuberculosis screening on or within 7 days prior to the first day of employment and free from Tuberculosis
  • The ability to obtain CPR/FA training within 60 days of hire and maintain a current certification
  • The ability to meet physical requirements for position
  • The ability to pass a drug test
  • Basic computer skills
  • Must be able to perform duties in accordance with the guidelines mandated by Social Services as the licensing agent of this facility
