About The Position

The Registered Dietitian will be responsible for coordinating the nutritional care of all Kroontje Health Care Center residents by completing an initial nutritional assessment and monitoring the residents’ evolving dietary needs. Central to success in this position will be a customer-service centered focus, as the Registered Dietitian will regularly participate in care planning meetings, offer nutritional counseling and education for residents and their family members, and interact on a daily basis with residents throughout the Center. Ultimately, the Registered Dietitian will be charged with providing services in accordance with all relevant local, state, and federal regulatory standards. Serve as the consultant dietitian for Showalter.

Requirements

  • A valid and active Registered Dietitian's license in the state of Virginia.
  • At least one year of experience working in a long-term care or hospital setting.
  • Excellent written and oral communication skills.

Responsibilities

  • Provide specialized nutrition care and support to all KHCC residents.
  • Provide nutrition consultation and education for residents and families with nutrition risk, identified with nutrition risk, and/or referred by physicians or other healthcare providers for nutrition-related problems, chronic disease management, management of nutrition support, to meet regulatory requirements, or for prevention and/or to promote wellness.
  • Ensure that each resident’s nutrition needs are being met, including when nutrition support initiation and/or management is required.
  • Provide education to residents and family members to help them voluntarily manage or modify food choices and eating behavior to improve or maintain overall health.
  • Implement needed nutritional interventions, including communicating the plan of nutrition care to staff, residents, and family members.
  • Uses monitoring and evaluation techniques that consider the resident’s cultural, religious, and ethnic concerns along with their nutritional needs.
  • Complete nutrition screening for new residents within 24 hours of admission in accordance with local, state, and federal regulations.
  • Determine nutrition diagnosis.
  • Regularly monitor and evaluate resident’s progress and communicate that progress with other team members as needed, including: Nutrition Technician, Director of Nursing, etc.
  • Develop strong working relationships with administrative, nursing, and dining teams.
  • In collaboration with the Nutrition Technician, plan and correct menus for residents on specialized diets based on established guidelines, nutritional needs, and patient preferences.
  • Evaluate residents’ food intake from all sources, with sensitivity towards any cultural, religious, and ethnic preferences and concerns.
  • Conduct resident rounds to assess adequacy of nutrition intake and resident satisfaction with means and meal services.
  • Monitor and verify any changes in diet orders and/or preferences.
  • Conduct weekly tray assessments to ensure adherence to quality standards and diet prescription.
  • Attends and participates in care plan meetings for residents.
  • Document relevant, accurate, and timely information as gathered during nutrition care process in the resident’s electronic medical record in accordance with departmental standards.
  • Develop and teach nutrition-based in-services for Warm Hearth staff.
  • Assume accountability for all policies and procedures contained in the WHV Employee Handbook.
  • Exemplify Warm Hearth’s values in all interactions with fellow staff members and with external customers like residents, family members, and visitors.
  • Occasionally conduct education seminars for WHV residents.
  • Perform other related duties as assigned by the KHCC Administrator.
  • Adhere to all facility safety policies and procedures, including but not limited to: infection control, safety, and residents’ rights.
  • Participate in fire and disaster drills.
  • Identify safety hazards and emergency situations and initiate corrective action.
  • Document incidents and/or unusual problems according to established facility procedures.
