About The Position

PACE / Adult Day + Senior Living / Healthcare We are seeking a Registered Dietitian (RD) to support nutrition care and dining services across two healthcare settings in New Jersey. This role may be structured as either full-time or part-time, depending on candidate availability and experience. Full-Time Schedule Would Include: 3 days/week at an Adult Day Center / PACE Program in Jersey City, NJ 2 days/week at a Health Center within a Continuing Care Retirement Community (CCRC) in West Caldwell, NJ This is an excellent opportunity for a dietitian who enjoys working with older adults, collaborating with interdisciplinary teams, and making a direct impact on clinical outcomes, resident quality of life, and dining experience. Position Summary The Registered Dietitian is responsible for providing nutrition assessment, care planning, documentation, and ongoing nutrition support for participants/residents in accordance with physician orders, regulatory requirements, and organizational standards. The RD will work collaboratively with nursing, providers, therapy, social services, and dining services teams to ensure high-quality nutritional care, regulatory compliance, and individualized support for participants and residents. Requirements

  • Registered Dietitian (RD/RDN) credential required
  • Current registration with the Commission on Dietetic Registration (CDR) required
  • Current New Jersey dietitian licensure (or eligibility) preferred/required as applicable
  • Strong knowledge of: Medical nutrition therapy Therapeutic diets Nutrition documentation Older adult nutrition Regulatory and survey expectations
  • Strong communication, organization, and interdisciplinary collaboration skills

Nice To Haves

  • Prior experience in one or more of the following preferred: PACE Adult Day Health Skilled Nursing Long-Term Care Rehabilitation Senior Living Healthcare / Clinical Nutrition

Responsibilities

  • Complete nutrition assessments, reassessments, and follow-up evaluations
  • Develop and update individualized nutrition care plans
  • Monitor participants/residents for: Weight changes Nutritional risk Appetite and intake concerns Skin integrity concerns Hydration status Supplement needs
  • Provide recommendations related to: Therapeutic diets Texture modifications Oral nutrition supplements Food allergies and intolerances Meal accommodations and preferences
  • Participate in interdisciplinary care planning and team meetings
  • Review and support compliance with physician diet orders
  • Collaborate with food service and culinary teams to ensure menu appropriateness and nutritional adequacy
  • Assist with resident/participant and family nutrition education as needed
  • Support regulatory readiness and compliance for all applicable standards
  • Maintain accurate, timely, and compliant documentation in the medical record
  • Assist with menu review, sanitation, food safety, and nutrition-related quality initiatives as needed
  • Provide nutrition care and support for adult participants enrolled in a PACE program
  • Work closely with interdisciplinary care team members to support participant health, independence, and quality of life
  • Provide nutrition care for residents in the healthcare center setting
  • Support clinical nutrition needs, dining service coordination, and regulatory compliance within a senior living / post-acute care environment
