About The Position

Oak River Rehab is seeking a dedicated Registered Dietitian (RD) to join their interdisciplinary team. The role focuses on improving resident health outcomes through expert nutritional assessments, care planning, education, and oversight of dietary services. The ideal candidate is passionate about clinical nutrition, resident-centered care, and collaboration within a healthcare team.

Requirements

  • Registered Dietitian with the Commission on Dietetic Registration (CDR) or eligible to sit for the RD Exam
  • Licensed Dietitian in the State of California (or eligible as required)
  • Bachelor's Degree in Food and Nutrition or related field
  • Maintain all required licenses, certifications, and continuing education
  • Remain in good standing with the California Department of Public Health

  • One year of clinical healthcare or skilled nursing experience
  • Completion of an accredited dietetic internship

Responsibilities

  • Complete nutritional initial, quarterly, annual, and significant change assessments in accordance with federal and state regulations.
  • Conduct monthly nutrition reviews for high-risk residents, including those with significant weight loss or gain, tube feeding, and hemodialysis.
  • Develop, implement, and monitor individualized nutrition care plans.
  • Assess nutritional needs and diet restrictions, and provide nutritional counseling.
  • Coordinate nutrition services with the Dietary Supervisor and interdisciplinary care team.
  • Monitor dietary operations for compliance with nutrition, food safety, sanitation, and quality standards.
  • Inspect diet trays to ensure physician diet orders are accurately followed.
  • Consult with physicians, nurse practitioners, dialysis dietitians, and other healthcare providers regarding resident nutritional needs.
  • Participate in daily stand-up meetings and interdisciplinary care conferences.
  • Provide leadership and support to the Dietary Department while promoting excellence in resident care.
