About The Position

The on-call Registered Dietitian will work in the Integrated Care Center Clinic, a primary care clinic primarily serving patients experiencing homelessness, poverty, and addiction. This role involves collaboration with medical and behavioral health staff to provide nutrition consultation (in-person or remote) for patients identified as at-risk for nutrition-related issues, chronic disease management, prevention, and wellness. The dietitian will function as part of a multidisciplinary team to educate and assist patients in achieving healthy eating goals, understanding nutrition programs, and improving health outcomes for conditions such as diabetes, hypertension, oral care, substance use disorders, mental health, and obesity. Key functions include performing nutrition assessments, developing and implementing evidence-based interventions, and monitoring patient progress through the nutrition care process.

Requirements

  • Bachelor’s degree or equivalent.
  • Registered Dietitian Nutritionist (RDN) credential.
  • Experience teaching adults and/or facilitating workshops.
  • Knowledge of nutrition programs, resources, and assistance.
  • Comprehensive knowledge of evidence-based nutrition therapy and techniques for effective behavior change.
  • Excellent verbal and written communication, planning, and time management skills.
  • High degree of creative energy and the ability to adapt easily to change.
  • Mastery of MS Word, Excel, PowerPoint, and Internet applications.

Nice To Haves

  • 1-2 years clinical experience desired.

Responsibilities

  • Conducts nutrition assessments using dietary history, medical history, BMI, and clinical data to evaluate current nutrition status.
  • Reviews dietary intake to identify factors affecting health conditions and nutritional risk.
  • Prioritizes nutrition problems for optimal patient health, recovery, chronic disease management, or disease prevention.
  • Formulates action plans for nutritional risks and problems using evidence-based nutrition practice guidelines.
  • Assesses education needs and provides nutrition counseling tailored to individual needs, considering medical, economic, social, religious, cultural, and ethnic factors.
  • Monitors and evaluates patients’ nutrition needs, measures outcomes, and adjusts plans as needed.
  • Provides health education sessions to individual patients, families, and groups.
  • Educates patients and staff on nutrition access programs (e.g., WIC, CalFRESH, food pantries) and community resources for healthy food access.
  • Creates and provides nutrition education handouts and recipes for patients.
  • Works in coordination with the multidisciplinary team (medical, dental, mental health, psychiatry, addiction medicine, and case managers) to identify and address nutritional needs.
  • Participates in case conferencing at provider and integration meetings to ensure quality health outcomes.
  • Transitions visits from nutrition-only to primary care visits as appropriate.
  • Identifies and engages patients with chronic conditions who can benefit from nutrition interventions to impact overall health goals.
  • Maintains accurate records in Epic.
  • Develops and maintains up-to-date nutrition materials for patient education.
  • Meets productivity and compliance guidelines of the program.
  • Generates reports as directed.
  • Performs other duties as assigned.
