Registered Dietitians telephonically coordinate wellness and disease management, with the focus on diabetes, asthma, COPD, CAD, CHF, Atrial Fib, HTN and hyperlipidemia, but not limited to this list. The Registered Dietitian (RDN) is responsible for providing comprehensive nutrition care and case management services to individuals with complex medical and social needs. This role integrates evidence-based nutrition practice with case management principles to support optimal health outcomes. The RDN collaborates with interdisciplinary teams, patients, families, and community resources to ensure coordinated, patient-centered care. The RDN conducts comprehensive nutrition assessments to evaluate members’ health status, dietary habits, and clinical risks, and develops individualized care plans aligned with medical conditions, provider recommendations, and evidence-based guidelines. The RDN delivers telephonic nutrition counseling and education to promote behavior change, improve self-management, and support long-term health outcomes. Use claims processing tools to review and research paid claim data to develop a clinical picture of a member’s health; create and maintain an individualized care plan for each patient; and provide ongoing follow-up with members actively engaged in care management. Provide guidance related to nutrition and medication interactions, reinforcing provider treatment plans and minimizing side effects. Support chronic disease management initiatives by addressing key health indicators, including A1c control, weight management, and cardiovascular risk factors. Work with patients to ensure they have a primary care provider; assist in referrals to specialists if needed; and support access to care by assisting in obtaining durable medical equipment (DME) and reviewing pricing options for high-cost medications. Collaborate with interdisciplinary care teams, including physicians, nurses, and case managers, to ensure holistic and coordinated care delivery across the continuum. Document interventions and patient contact; maintain privacy of member’s records and assist in providing quarterly reports to clients. Track groups of high-risk members and associated interventions to demonstrate measurable improvements in overall population health outcomes. Be able to meet established productivity, quality, and turnaround time (TAT) requirements on a daily, weekly, and monthly basis. Demonstrate the ability to meet productivity, quality, and turnaround expectations consistently. Maintain readiness for and successfully pass external audits, including URAC and NCQA accreditation standards. Maintain HIPAA compliance in accordance with company policies and procedures, ensuring strict confidentiality and adherence to minimum necessary requirements. Complete all required annual training within designated timelines and maintain professional competency through ongoing education.
