About The Position

CentroMed is seeking a Registered Dietitian to join their team. This role is integral in health behavior and interventions for chronic disease prevention and management. The Registered Dietitian will assess, evaluate, implement, and document nutritional interventions for Chronic Disease Prevention Program participants. This includes providing nutritional assessments and information through individual consultations and group classes, while maintaining knowledge of program and agency safety, infection control, OSHA, and Joint Commission standards.

Requirements

  • Minimum BS in Nutrition or related field.
  • Registered Dietitian credential required.
  • Prior experience in nutritional assessment and planning.
  • Two (2) years’ experience in a health care facility.
  • Registered with the Commission on Dietetic Registration.

Nice To Haves

  • Prior post-acute care experience preferred.
  • Bilingual (Spanish) preferred.

Responsibilities

  • Collaborate with primary care providers.
  • Document information in the patient chart, implement a plan, provide nutrition counseling, and make appropriate referrals.
  • Conduct monthly performance assessments in compliance with quality assurance activities.
  • Regularly use Electronic Health Records (EHR) and systems for patient care, communication with providers, partners, and administration.
  • Coordinate follow-up schedules for the caseload.
  • Consult with patients and families regarding nutrition needs and goals.
  • Input dietary information accurately and in a timely manner.
  • Exhibit excellent customer service and a positive attitude toward patients.
  • Follow all agency and program protocols to maintain compliance with OSHA regulations and Joint Commission.
  • Provide nutrition counseling to program participants.
  • Utilize motivational interviewing counseling techniques during counseling sessions.
  • Develop a referral network of community resources to address patient needs.
  • Be knowledgeable on the impact of nutrition assessment as a preventable measure for chronic diseases.
  • Perform all other duties assigned by the Director - Chronic Disease Prevention Program.
  • Abide by infection control policies and procedures.
  • Abide by policies and procedures as dictated in the Employee Safety Handbook.
