About The Position

Carolina SeniorCare is a P.A.C.E. (Program of All-inclusive Care for the Elderly). A career at Carolina SeniorCare is a calling to help others in their community and help elderly participants (55 or older) stay and live in their respective communities. P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs! Under the supervision of the Center Administrator, the Registered Dietitian screens and assesses participant nutritional status for Carolina SeniorCare. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for Carolina SeniorCare participants requiring interventions. Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with contracted meal services to ensure meals meet the needs of the Carolina SeniorCare participants. Provides education to other health care professionals as appropriate. Provides direction and supervision to Food Services Supervisor.

Requirements

Bachelor’s or advanced degree from an accredited college with major studies in food and nutrition or dietetics required.
Be legally authorized, currently licensed, registered or certified if applicable to practice in the State of North Carolina.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Must have a valid driver’s license, proof of insurance and have means of transportation.
Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.
CPR/First Aid certification required.
Minimum of one year working with the frail or elderly.

Responsibilities

Conducts an initial comprehensive nutritional assessment on all Carolina SeniorCare participants upon enrollment.
Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
Conducts and an in-person reassessment minimally on an annual basis or as needed.
Functions as a member of the Interdisciplinary Team.
Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
Monitors monthly weights, medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes hydration status, intake, skin breakdown, reports changes to the Interdisciplinary Team and makes recommendations for changes to appropriate staff.
Documents nutrition notes and all other pertinent activities in the participant’s medical record according to Carolina SeniorCare standards.
Provides nutritional counseling and education for Carolina SeniorCare program participants and or caregivers using appropriate materials.
Provides nutritional educational programs to own department or other departments/Carolina SeniorCare staff.
Assists in monitoring meals, including intake, diet compliance and tray accuracy.
Develops and implements the nutritional components of participant plan of care including active participation and development of therapeutic dining programs.
Maintains supplement/snack/bag lunch lists and dietary census for Carolina SeniorCare participants.
Directs, supervises, performs initial and annual competencies on, and evaluates the performance of the Food Services Supervisor.
