About The Position

Colorado PACE has an immediate opening for a Registered Dietitian. The purpose of this position is to consult with provider and team members to determine nutritional needs and dietary modifications for participants. Provides instruction to participants, their families, and housing partners in nutritional principles, dietary plans, food selection, and preparation. This position is responsible for working with participants and their families to determine appropriate methods of seasoning food to meet the participants’ preference while staying consistent with their dietary restrictions as set by their provider.

Requirements

  • Bachelor’s Degree from an accredited college with major studies in nutrition, dietetics, or related field.
  • One year experience working with elderly population, home care or geriatric experience.
  • Ability to work effectively with culturally, economically, and educationally diverse populations and form positive interpersonal relations in dealing with a wide range of staff participants is essential.
  • Must be a Registered Dietitian through the Commission on Dietetic Registration.
  • Valid Colorado Driver’s License and proof of auto insurance.

Responsibilities

  • Provides for continuity of service across care settings through effective information management, including both verbal and written communication.
  • Involves the participant/family in setting nutrition and dietary goals and objectives.
  • Participates in the orientation of new members of the interdisciplinary team, volunteers, and students.
  • Works with participants and their families to determine the best method for seasoning participant’s food to meet the participant’s goal for taste.
  • Completes initial assessment, periodic assessments, annual assessments, and initiates change in plan of care as appropriate to meet the needs of the participant and family.
  • Plans specific interventions in collaboration with the interdisciplinary team, as appropriate, to resolve current problems and prevent potential problems.
  • Provides education related to PACE participant’s nutritional needs and status in alternate care settings (nursing homes, hospitals, assisted-living facilities, etc.).
  • Identifies need for intervention by other members of the interdisciplinary team on an ongoing basis; collaborates in the planning of such care.
  • Participates in the IDT and communicates changes in the participant’s condition not limited to in-home nutrition needs, eating habits, and weight issues.
  • Acts independently and responsibly to perform job duties on a consistent basis.
  • Manages nutritional supplements and special dietary needs/restrictions.
  • Provides clinical oversight of dietary services for the organization.
  • Carries out the mission and values of Colorado PACE.
  • Respects the autonomy and right to confidentiality, self-determination, informed consent, privacy, and security of all participants/participants and families.
  • Evaluates the effectiveness of care in conjunction with other members of the interdisciplinary team.
  • Reports changes in participant condition to the provider and seeks needed changes as warranted.
  • Documents care provided and the response of the participant/family timely in the participant’s medical record.
  • Participates in the development of policies, procedures, clinical protocols, forms, and service programs as requested.
  • Participates in departmental and/or agency-wide quality improvement activities.
  • Manages a caseload and delivers care in a cost-effective manner that demonstrates an understanding of reimbursement methodologies.
  • Manages the food program at the Center.
  • Monitor compliance with the approved agency budget, including staffing and direct patient care costs.
  • Monitors and responds to reports including utilization of goods and services.
  • Utilizes company resources in a prudent and appropriate manner.
  • Is a good steward of company resources.
  • Performs other duties as assigned.
