Responsibilities
- Conduct comprehensive nutrition assessments and provide individualized nutrition consultations.
- Develop and implement personalized nutrition care plans.
- Monitor patients' nutritional status and recommend appropriate interventions.
- Collaborate with interdisciplinary healthcare teams to optimize patient care.
- Support quality improvement initiatives and maintain regulatory compliance readiness.
- Improve patient outcomes by leveraging a Food as Medicine approach.
