BoldAge PACE is an all-inclusive program of care, personalized to meet the individual health and well-being needs of our participants. Our approach is simple: We listen to our participants and their caregivers to truly understand their needs and desires. This role assesses nutritional status of PACE participants, plans and implements appropriate nutrition interventions, and communicates this information to the Interdisciplinary Team to ensure participants' nutritional needs are met. The Registered Dietitian functions as a member of the Interdisciplinary Team, providing nutrition counseling, education, and therapy to participants, as well as educating and counseling family and other caregivers regarding dietary issues. They conduct initial assessments and periodic reassessments, participate in the development of the plan of care, assist in coordinating 24-hour care delivery, and regularly inform the Interdisciplinary Team (IDT) of each participant's nutritional needs. They also provide education to other healthcare professionals as appropriate.
