About The Position

Responsible for coordinating treatment and implementing services for assigned clients as a member of a multi-disciplinary Early Childhood Services team. This role involves providing outreach, conducting nutrition assessments, developing nutrition plans, and offering education and counseling to families and staff. The Registered Dietitian will also consult with team members, maintain accurate client records, complete billing documentation, and adhere to professional standards and regulations. Additionally, the role includes conducting client visits in natural environments, leading or co-leading groups, developing group curricula, and participating in team meetings and program development.

Requirements

Master's Degree in Nutrition
Current licensure as a Registered Dietitian/Nutritionist by the Connecticut Board of Registration /Division of Professional Licensure.

Responsibilities

Provide outreach services to children and families in relation to nutrition related issues.
Conduct nutrition assessments, review individual child and family nutrition and develop individual nutrition plans.
Provide nutrition education, training and counseling to families and staff.
Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients.
Maintain client records completely and accurately to include IFSP’s with full assessment of client functioning and goals, documentation of client progress in the program, records of required contact with collaterals and family members, documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clients.
Complete billing documentation according to schedule established by the company.
Adhere to professional standards and guidelines according to their respective disciplines.
Keep supervisor informed of progress of all clients.
Meet direct service requirements based on part time or full time status, as designated.
Adhere to provisions of M.G.L. Chapter 119, Section 51A (Mandated Reporter); report all occurrences immediately to the clinic.
Adhere to clinical recommendations from clinical administration.
Maintain confidentiality in all clinical matters.
Remain compliant with billing and documentation in accordance with direct service requirement.
Conduct visits with clients and relevant collaterals in natural environments according to the needs of the child and family.
Lead or co-lead groups within Early Childhood standards according to program needs and design (both child and parent focused).
Develop group curricula for assigned groups as applicable.
Participate in team meetings, supervision and in-service training as required.
Participate in program development and team goals.
