Provides nutritional support to participants/members in the Baptist Health and Wellbeing program assisting in the development of nutritional strategies to improve the health status of defined populations. Serves as a nutritional advisor, advocate, and educator to program participants in the achievement of specific chronic disease management objectives to improve health outcomes and status through nutritional assessment, consultation, and education. Utilizes a number of population health management tools and analytics to design education and resources so that program members are actively involved in the management of their own health and wellbeing. Works in coordination with the Baptist’s Health and Wellbeing professional team and other health care providers, as needed, to monitor care coordination efforts for both incremental and long-term improvements. Performs other duties as assigned.
