About The Position
Evergreen Post Acute is hiring a full-time Registered Dietitian to join the clinical team at our 151-bed skilled nursing and rehab community in Smyrna. This role involves completing nutrition assessments, developing care plans, participating in interdisciplinary care conferences, and documenting in PointClickCare.
Requirements
- Active RD/RDN credential in good standing
- Delaware LDN license, or eligibility to obtain one
- Bachelor's degree or higher from an ACEND-accredited program
Nice To Haves
- Long-term care experience
- Familiarity with MDS, PDPM and NFPE
Responsibilities
- Complete admission, quarterly and significant-change nutrition assessments for post-acute and long-term residents
- Develop individualized care plans
- Participate in interdisciplinary care conferences and weekly risk meetings
- Contribute nutrition sections for MDS and support accurate PDPM capture
- Document in PointClickCare
