About The Position

Grace Community Care & Homes is developing its Diabetes and Nutrition department and seeks a Registered Dietitian Nutritionist. The role contributes to person-centered services, reliable operations, and accurate documentation. The position is planned, and its start date and duties are contingent on applicable program approvals, funding, credentialing, and staffing readiness. Services may begin only within the approved scope and at approved locations.

The position is based in New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. It is part-time; assignment and weekly schedule will be discussed during hiring. Final regulatory eligibility will be verified before appointment or service delivery. Requirements

• Current Registered Dietitian Nutritionist (RDN) credential and applicable state requirements.
• Eligibility for the selected medical nutrition therapy (MNT) payer enrollment.
• Clinical nutrition experience.
• Applicants should provide relevant credentials, experience, availability, and a resume.

Responsibilities

• Provide nutrition assessments, individualized medical nutrition therapy, education, and follow-up.
• Document outcomes and coordinate with treating clinicians.
• Work collaboratively with department leadership, intake, credentialing, billing, and quality teams.
• Maintain confidentiality, respect participant choice, and promptly communicate service gaps.
• Follow role-specific policies and professional scope.
• Participate in orientation, competency review, and quality improvement.

Benefits

• Pay of $32–$40 per hour, based on qualifications and relevant experience.
