About The Position

CareOne at Brookline is hiring a Full-Time Registered Dietician for its Long Term Care, Short Term Rehab, and Skilled Nursing facilities. This role is responsible for coordinating the nutritional care of residents and patients, including completing nutritional assessments, participating in care planning meetings, and providing nutritional counseling and education to residents and their families. The position requires meeting DOH/state/federal and company standards and performing duties independently.

Requirements

Must possess Registered Dietician licensure within the state of operations
High comfort level with enteral nutrition
At least one year of experience in a long-term care setting
Experience with MDS 3.0
Strong oral and written communication skills
Familiarity with state survey process and compliance to nutrition care regulations

Nice To Haves

Experience with electronic charting and the Nutrition Care Process preferred

Responsibilities

Coordinate nutritional care of residents and patients by completing nutritional assessments
Participate in care planning meetings as required
Provide nutritional counseling and education for residents and family
Meet DOH/state/federal and company standards
Perform duties independently without training or direction
