About The Position

Oakwood Village is seeking Registered Nurses (RNs) to join their organization, which has been recognized for its excellence in retirement and nursing care. The organization has recently expanded its services to include home health and hospice and opened a new skilled nursing facility. This role involves making a meaningful difference in the lives of older adults by participating in the assessment, planning, implementation, and evaluation of resident care. The RN will collaborate with other healthcare professionals and lead nursing assistants. Requirements

  • Registered Nurse License in the state of Wisconsin required for all RN positions
  • CPR Certified, required

Nice To Haves

  • One year relevant nursing experience, preferred
  • Knowledgeable in geriatric care, preferred

Responsibilities

  • Participate in the assessment, planning, implementation, and evaluation of resident care.
  • Collaborate with nurses from all shifts, interdisciplinary staff, and physicians on resident care management and coordination.
  • Provide direct resident care.
  • Lead MAA, CNA, and RA staff.
  • Perform resident care such as medication administration, treatment, therapy, basic teaching and reinforcement education.
  • Contribute to the assessment, planning, implementation, evaluation and documentation of resident conditions and care.
  • Document and report changes in condition, intervention and resident response.
  • Update Care Plan consulting with floor RN as appropriate.
  • Communicate Care Plan changes with Nursing staff/Director of Nursing Care.
  • Attend Care conferences as designated.
  • Report changes in resident condition to supervisor and seek guidance from RNs as needed.
  • Deliver nursing care in compliance with Nurse Practice Act, State and Federal regulations applicable to SNF setting and policies and procedures of Oakwood Village.
  • Collaborate with nursing team, medical consultants, and other team members to ensure resident and family needs are met.
  • Demonstrate effective time management and problem-solving skills.
  • Perform other duties as assigned.
