About The Position
This role serves as the front door for Access’ Behavioral Health and Substance Use Services, assessing and following clients from initial presentation and providing acute care until connection with ongoing care. The position involves providing individual, family, and group clinical services to support children, adolescents, adults, and families experiencing life stressors or acute behavioral challenges, utilizing Evidence-Based Clinical Practices. Training and clinical supervision will be provided for qualified professionals seeking hands-on experience with a variety of Evidence-Based Practices.
Requirements
- NY State RN License and minimum 1-year experience
- Experience working with diverse populations and crisis intervention
- Commitment to customer service excellence and a warm and engaging presence
- Computer skills and experience working with electronic medical records
Nice To Haves
- Psychiatric and Substance use disorder experience is a plus
- Experience working with people diagnosed with I/DD is a plus
- Bilingual English and Spanish speaking is a plus
Responsibilities
- Assess and triage patients appropriately based on their acuity.
- Conduct assessments through interviews with patients and relatives to gather case history.
- Provide general nursing care to patients in accordance with nursing standards.
- Consult with psychiatrists and other behavioral health staff in designing appropriate treatment options.
- Administer prescribed medications and treatments in accordance with nursing standards.
- Assess, observe, record, and report patient’s condition and reaction to treatment to the team.
- Educate patient/family about procedures, medications, nutritional maintenance of health and wellness.
- Act as a patient advocate.
- Assist in maintaining a safe and secure environment for staff and patients.
- Coordinate with the interdisciplinary team in planning the care of the patient and discharge planning, including follow-up and outreach after discharge.
- Maintain examination rooms for necessary supplies and equipment.
- Ensure cleanliness in the worker area, keeping it neat and orderly.
- Prepare supplies and equipment and assist providers during examinations and treatment.
- Perform other duties that may be assigned from time to time.
