About The Position

MedOne is seeking an experienced Regional Clinical Resource Nurse Practitioner / Physician Assistant to provide coverage across multiple post-acute care facilities throughout Central Ohio. This role offers variety, autonomy, and collaboration, with the opportunity to make a meaningful impact where needed. The Regional Post-Acute APP serves as a clinical resource, ensuring continuity of care by providing coverage during provider vacations, leaves of absence, onboarding of new facilities, open territories, and periods of increased clinical demand. The role involves practicing across diverse clinical environments, collaborating with physicians and interdisciplinary teams, and adapting to various workflows and EMR systems. Travel is generally limited to facilities within a 60-minute drive from the clinician's home base, with mileage reimbursement. In addition to direct patient care, this clinician may support telemedicine, palliative care, value-based care initiatives, quality improvement projects, and provider education.

Requirements

• Graduate of an accredited Physician Assistant or Nurse Practitioner program (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner).
• Current national certification as a Physician Assistant or Nurse Practitioner.
• Current, unrestricted Ohio licensure as a Physician Assistant or Advanced Practice Registered Nurse (APRN).
• Current DEA registration and prescriptive authority in accordance with Ohio law, as applicable.
• Active OARRS account.
• Eligible for privileges at post-acute care facilities.
• Minimum of one year of post-acute, skilled nursing, rehabilitation, hospital medicine, geriatrics, or long-term care experience required.
• Strong clinical assessment, diagnostic, and decision-making skills.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating effectively with physicians and interdisciplinary teams.
• Adaptability to multiple facility workflows, electronic medical record systems, and clinical environments.
• Valid driver's license with reliable transportation and willingness to travel throughout the assigned service area.

Responsibilities

• Provide comprehensive medical care to new and established patients across multiple MedOne post-acute care facilities throughout Central Ohio.
• Perform comprehensive histories, physical examinations, and interpret laboratory and diagnostic studies for medically complex post-acute patients.
• Develop, implement, and adjust individualized treatment plans in collaboration with supervising or collaborating physicians.
• Order, interpret, and communicate diagnostic and laboratory testing.
• Prescribe pharmacologic and non-pharmacologic therapies in accordance with Ohio law and scope of practice.
• Monitor patient progress and modify treatment plans as clinically indicated.
• Coordinate care with nursing staff, therapists, specialists, consultants, hospice and palliative care providers, patients, and families.
• Facilitate admissions, hospital transfers, discharges, readmissions, and other transitions of care.
• Participate in interdisciplinary care conferences, family meetings, and discharge planning.
• Maintain accurate, timely clinical documentation and complete billing in accordance with MedOne, facility, and regulatory requirements.
• Maintain all credentialing requirements and clinical privileges.
• Provide clinical coverage for provider vacations, PTO, leaves of absence, open territories, newly contracted facilities, and unexpected staffing needs.
• Support onboarding and workflow implementation for newly contracted facilities.
• Travel to MedOne partner facilities generally located within a 60-minute one-way drive from the clinician's designated home base.
• Adapt efficiently to multiple facility workflows, interdisciplinary teams, electronic medical record systems, and operational processes.
• Complete telemedicine visits when clinically and operationally appropriate.
• Support Accountable Care Organization (ACO) initiatives, including Annual Wellness Visits, Transitional Care Management, chronic disease management, readmission reduction, quality measures, and other value-based care initiatives.
• Provide palliative care consultations and follow-up visits as operational needs allow.
• Participate in APC call coverage as assigned to support continuity of patient care.
• Assist leadership with quality improvement initiatives, provider education, onboarding of new facilities, workflow implementation, and special projects.
• Communicate timely clinical handoffs to the primary provider, including significant patient changes, pending results, follow-up needs, and ongoing plans of care.
• Serve as a clinical resource and positive ambassador for MedOne by promoting teamwork, professionalism, adaptability, and excellence in patient care.
