The Regional Manager, Ambulatory Pharmacist is a patient-centered clinical leader dedicated to optimizing medication therapy within primary care clinics. Operating under a collaborative practice agreement, this role focuses on reducing polypharmacy, enhancing medication adherence, and managing chronic diseases to improve patient outcomes and elevate quality performance metrics such as CMS Star Ratings and HEDIS scores. The manager partners closely with patients, physicians, advanced practice providers, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness. This role includes approximately 3.5 days per week dedicated to direct patient care and 1.5 days allocated to administrative responsibilities, including oversight and management of pharmacists within the assigned multistate region. The schedule is expected to follow a cadence of 3 weeks focused on direct patient care, followed by 1 week of travel supporting ambulatory care pharmacists across the region.

Requirements

  • Knowledge of and experience with state and federal insurance programs
  • Familiarity with appropriate local, state, and federal pharmacy-related regulations
  • Ability to work effectively both independently and as part of a multidisciplinary team
  • Excellent leadership, organizational, communication, and clinical decision-making skills
  • Strong attention to detail and ability to manage multiple priorities
  • Willingness to travel regionally up to 50%; flexible to work evening, weekends and/or holidays as needed
  • Fluency in spoken and written English
  • Doctorate of Pharmacy (PharmD) required
  • Valid pharmacy license in good standing (Doctor of Pharmacy/PharmD) required
  • Minimum of five (5) years of experience in primary care, ambulatory care, managed care, or related clinical setting with at least two (2) years of pharmacist management experience
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment
  • This position requires possession and maintenance of a current, valid Driver's License

Nice To Haves

  • BCACP or BCGP a plus
  • Residency training or ambulatory care certification is highly preferred
  • Certification in immunization and medication therapy management is a plus
  • Experience in managed care, HMO, PBM, or MSO settings is a plus

Responsibilities

  • Direct supervision of Ambulatory Pharmacists and support staff, including hiring, performance reviews, and development plans within their region.
  • Program Oversight — Manages the ambulatory pharmacy program within their region and their own location/center.
  • Quality & Metrics Leadership — Owns CMS Star Ratings and HEDIS performance at a regional level, including A1C and HBP, not just contributes to them, as well as enhancing medication related STARs impacts.
  • Operational Accountability — Directs patient care and outcomes, scheduling, policy development, management, regulatory compliance, and EMR coordination.
  • Stakeholder Leadership — Primary liaison to physician, payer relations when necessary, population health, and senior administration.
  • Education & Precepting — Responsible for staff development for ambulatory care pharmacists within the region.
  • Medication Management & Optimization: Collaborates with healthcare providers to adjust medication therapies, interpret lab results, and support clinical decision-making within a protocol-driven framework.
  • Comprehensive Medication Reviews: Conducts detailed evaluations of medication regimens addressing safety, efficacy, cost considerations, side effects, and barriers to adherence.
  • Chronic Disease Management: Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications.
  • Care Coordination & Transitions: Facilitates seamless transitions of care by resolving medication discrepancies during hospital discharges and preventing readmissions.
  • Patient Education & Engagement: Delivers personalized counseling on medication use, side effects, and adherence strategies to empower patients and improve compliance.
  • Lab Testing & Monitoring: Orders, interprets, and utilizes laboratory results to monitor medication, safety and effectiveness.
  • Collaborative Practice & Protocols: Works directly with physicians and providers to implement and follow care protocols, including independent medication adjustments when appropriate.
  • Administrative & Support Activities: Assists with physician consultations, provides accurate drug information, and supports the department’s quality improvement initiatives.
  • Performs other duties as assigned or modified at manager’s discretion.
